To Be Done Prior To Admissions
PRE-ADMISSIONS MEETING: this allows for you and your family to obtain a brief view of the treatment program, and what to expect while your child is in treatment at the Bobby Benson Center (BBC). Pre-admissions meeting participants should include (but not limited to): you and your child, Parole Officer (if applicable), assigned Care Coordinator (if applicable), and BBC Staff: Clinical Director, Case Manager and Intake Coordinator.
REQUIRED MEDICAL DOCUMENTS: Once youth has been approved for intake, it is required that the following be submitted, preferably two weeks but absolutely no later than 1 week prior. Failure to submit these items on time may result in a delay of admittance into treatment.
Documentation of Tuberculosis (TB) Clearance dated within one year of youth's admission
Documentation that the youth has had a Physical exam completed by a physician dated within one year of youth's admission
The Medical Intake packet must be completed and submitted to the intake coordinator. This will help ensure that there is no interruptions in the youth's medical/medication care. A complete list of all required pre-admission paperwork/items can be found HERE.
Also, upon admission to the Bobby Benson Center, parents and/or authorized guardian will need to provide clothing & hygiene supplies (see "What To Bring Upon Admission" section).
