HEALTH INSURANCE COVERAGE
Bobby Benson Center currently accepts the following major health insurance carriers: HMSA, Kaiser Permanente, UHA/Optum, Hawaii Laborers Union, MedQuest. To ensure a smooth intake and payment process it is imperative that all insurance information provided to the center is current and accurate (Provider name, Medical Number and is not terminated).
It is the sole responsibility of the youth's Legal Guardian to contact their insurance provider and verify the following information: Youth's coverage is still effective, whether the plan covers substance abuse/residential treatment, what the co-pay responsibility is according to the benefits of their plans, what their co-pay maximum is and how much of it has already been met. Please use the Insurance Verification and Waiver Form located on the Required Documents page HERE.
Once this information has been obtained the Legal Guardian can reach out to Bobby Benson Center's Accountant at (808) 376-3015 to discuss payment plans/options further.
Bobby Benson Center also has a contract to provide services to youth who are part of the Child and Adolescent Mental Health Division (CAMHD) services. Your youth may qualify for CAMHD service. To learn more please click HERE. If the Youth is currently uninsured and qualifies for CAMHD service please call our intake coordinator to get the application process started!
